Terms & Conditions

Evolve Midwifery Services

 

At Evolve Midwifery Services, we aim to provide respectful, evidence-based, and individualised care.
These Terms & Conditions are here to create clarity, shared understanding, and a supportive framework for our work together.

By booking or attending an appointment, you agree to the following Terms & Conditions.

1. Scope of services

Evolve Midwifery Services provides:

  • Midwifery care and education

  • Lactation and infant feeding support

  • Antenatal and postnatal consultations

  • In-clinic appointments, home visits, and phone consultations

Our services are not an emergency service.
If you or your baby require urgent medical care, please contact your GP, maternity unit, or emergency services.

2. Professional standards

All care is provided in accordance with:

  • Professional and ethical obligations of AHPRA

  • The Nursing and Midwifery Board of Australia standards

  • Evidence-based practice and informed consent principles

Care is collaborative and supportive. Decisions remain yours.

3. Appointments & time selection

Clients are responsible for selecting an appointment length that allows sufficient time for their needs.

As a general guide:

  • Initial postnatal breastfeeding support usually requires 90 minutes

  • Initial mastitis appointments typically require 45–60 minutes

  • Antenatal initial sessions are 60 minutes, unless a longer session is booked

If additional time is required, extra fees may apply.

4. Fees & payment

Fees are due at the time of your appointment.

Current fees:

  • 30 minutes – $180

  • 60 minutes – $250

  • 90 minutes – $350

Appointments that run beyond the booked time incur an additional $50 per 15 minutes.

5. Home visits & travel fees

Home visits attract:

  • A $20 travel fee within 20 km of Deakin

  • Longer distances are charged at $1 per kilometre, in addition to the $20 travel fee

​

6. Phone calls & follow-up

Phone consultations are available where clinically appropriate.

  • Short follow-up phone calls (up to 10 minutes) may be bulk billed for eligible clients with Medicare if the baby is under 7 weeks old

  • For older babies, phone consultations are charged at the equivalent of the Medicare rebate (currently $55 for up to 15 minutes)

     

    Longer phone consultations are charged based on their duration, in line with our appointment length fees. Medicare or other rebates may apply, where eligible.

     

Phone consultations are not suitable for urgent concerns.

7. Medicare, rebates & billing

  • Medicare rebates may apply until your baby is 6 weeks and 6 days old

  • Private health fund rebates depend on your individual policy

  • Defence referrals may be direct billed where applicable

Clients are responsible for understanding their own rebate eligibility.

8. Cancellations & missed appointments

If you need to cancel or reschedule, please text 0423 109 559 as soon as possible.

  • More than 24 hours’ notice – no fee

  • 12–24 hours’ notice – 25% of the consultation fee

  • Less than 8 hours’ notice – 50% of the consultation fee

Compassion grounds are always considered for circumstances beyond your control.

9. Late arrivals

If you arrive late, your appointment may need to finish at the originally scheduled time to avoid delays for other clients. Full fees may still apply.

10. Client responsibilities

Clients are responsible for:

  • Providing accurate and relevant health information

  • Bringing requested documentation (e.g. pregnancy records, baby health record)

  • Communicating concerns or questions openly

  • Treating staff with respect and courtesy

  • ​

11. Privacy & confidentiality

Your personal and health information is handled in accordance with our Privacy Policy, Australian privacy legislation, and professional obligations.

12. Changes to services or terms

Evolve Midwifery Services may update services, fees, or these Terms & Conditions from time to time.
The most current version will always be available on our website.
